Health insurance

We offer two different plan options through HealthPartners Insurance Company. With the HealthPartners Open Access Plan, you receive care from a defined network of providers that includes over 19,000 physicians. The True Deductible Plan offers a lower bi-weekly premium, with a deductible of $500 Individual/$1,000 Family. After the deductible is met, the plan coverage is 80/20. The Co-Pay w/Deductible Plan has a higher bi-weekly premium with with a $25 co-pay for office visits, and a $250 Individual/$500 Family deductible that applies only to in-patient/out-patient hospital services. Under either plan, formulary prescription drugs are covered with a $12.00 co-pay for generic; $32 co-pay for brand name.

CareLine

With CareLine, you have an after hours resource staffed by nurses who are specially trained to help you over the phone with your urgent medical concerns.

BabyLine

BabyLine is staffed by obstetric nurses who can help with questions relating to; pregnancy, new baby care, nursing, and postpartum concerns.

Dental insurance

Preventive, basic, major and orthodontia services through Delta Dental of Minnesota. Two different network provider options are available.

The Delta Preferred Option provides a smaller network of providers with richer benefits, including no deductible and an annual maximum of $2,000.

The Delta Premier Option provides a very large network of providers with slightly fewer benefits, including a deductible of $50 Individual / $150 Family.

Flexible spending account plan

All employees are able to pay for medical and dental insurance premiums, non-reimbursed medical and dental expenses and dependent care costs on a pre-tax basis, resulting in more take home pay.

Eligibility: All regular employees exceeding 32 hours per week, 1 st day of the month following 30 days of employment.

Life insurance

Term life insurance: coverage of 1x salary capped at $50,000 ; premiums are company paid.

Additional term life insurance up to 5-times salary may be purchased by the employee through payroll deduction.

Eligibility: All full-time employees, 1 st day of the month following 30 days of employment.

Disability insurance

You become eligible for disability insurance benefits if you are disabled and unable to work after 7 consecutive days. Through the 90th day of disability, the benefit is calculated at 60% of your weekly earnings up to a maximum of $1385 per week. After 90 days, the maximum benefit increases to $7,500 per month.Premiums are company paid.

Eligibility: All regular employees exceeding 30 hours per week, 1 st day of the month following 30 days of employment.

Employee savings/profit sharing(401k)

Pre-tax savings plan, with company matching contribution (40% on the first 6% of wages deferred by the employee) and profit sharing contribution.

Eligibility: Any employee who is 21 years of age and has completed 3 months of service.

Vacation

Twelve days per year for the first five years of service; Seventeen days per year for years of service six through ten; and Twenty-two days per year thereafter.

Eligibility: All full-time employees; accrues at the start of employment and is available after three months.

Holidays

The company provides 10 paid holidays per year to full-time employees and 5 paid holidays per year for part-time employees, working an average of at least 20 hours per week.

Eligibility: All full-time employees following 30 days of employment.

Sick time

Three days of sick time per year; available for absences due to illness/injury of the employee and immediate family members and visits to doctors and dentists.

Eligibility: All full-time employees; accrues at the start of employment and is available after three months.
